The Quality Manager (QM) is responsible for the development and overall management/maintenance of the Quality Assurance/Performance Improvement (QAPI) Program for the Home Health Agency.
The QM will objectively and systematically measure, monitor and assess program(s) operations and leads to measurable improvements in agency defined goals in the areas of:
patient safety, outcomes, care delivery and operations.
The QM will also be responsible for the direct oversight of the following HH programs of:
Infection Control, and Medication Management.
Responsible for the development, implementation and active management/monitoring of the Quality Assurance/Performance Improvement (QAPI) program.
The QAPI program will measure, analyze and track quality indicator data to assess operation and processes of care, including adverse patient events.
Data is used to monitor the effectiveness and safety of services, the quality of care, and to identify and act on opportunities for improvement.
Maintains current knowledge related to Medicare Conditions of Participation, CHAP, National Patient Safety Goals, Title 22, CAHPS reports, regulatory compliance and standards of practice.
Able to incorporate OASIS data into the QAPI Program.
Implement, monitor and evaluate performance improvement activities utilizing continuous quality improvement principles and methodologies including, but not limited to, LEAN and Six Sigma Principles.
Patient clinical record reviews are systematically reviewed on a bi-annual basis to assess appropriateness and adequacy of care and services provided as well as ensuring compliance with established policies regarding care delivery and professional standards of practice.
Patient data and input from patients and families is collected and analyzed to assess the quality of and trends of HH operations.
Data on accidents, equipment failures, incidents or other events having a disruptive impact, as well as adverse events to patients resulting from inadequate or malfunctioning equipment, items or services are reviewed and analyzed to assess performance.
Compiles, trends and reports quality data following the QAPI plan to include patient safety, patient and family experience, outcomes, quality of care, complaints, adverse events, potentially avoidable adverse events and errors.
Coordinates presentation of data to Management Team, PAC, QA committee and reports to HH governance, via Administrator, and leadership at least annually and information is available to HH Personnel.
Performs data analysis and identifies opportunities for improvement regarding hospital admissions and readmissions.
Participates in preparation for compliance survey and medical reviews as appropriate.
Develop, implement and maintain an educational program to meet the needs of all disciplines.
This will include QAPI training within the first 90 days of hire, and on an ongoing basis.
Educational requirements to also include annual competencies for clinical staff.
Other duties as assigned.
Requirements RN with current licensure to practice professional nursing in the state of California.
Bachelor's degree in nursing preferred.
Minimum 2 years experience in a health care environment.
Moderate to advanced knowledge of QAPI principles and methodologies.
Basic computer skills (Word, Excel) and interpersonal skills.
Able to manage self and others in a team-oriented, collaborative environment while meeting deadlines and achieving targets.
Valid California driver's license and proof of automobile insurance.
