Position Overview As an advanced practice clinician, you will be the face of Woundtech, providing direct patient care.
Each clinician is part of a regional team that consist of Regional Medical Directors, clinicians, patient care coordinators, case managers, and specialists for safety, quality, education and infectious disease.
You will be assigned patients within your geographic area and typically carry a panel of approximately 16-25 patients that you care for throughout the duration of care.
As patients are discharged, clinicians receive new Starts of Care cases to evaluate ongoing clinical needs and develop patient specific plans of care.
We believe in providing the best support for our clinicians, so you will receive extensive training in evidence-based clinical practice protocols specific to the various wound etiology and participate in weekly continuing wound care education.
In addition, Woundtech provides education and certification reimbursement for ongoing professional development.
All of this provides the basis for success as you consider all relevant local and systemic factors to guide wound care treatments which results in a more complete, patient-focused, sustainable wound management model.
From the developed patient plan of care, advanced practice clinicians perform procedures such as debridement and negative pressure device applications as well as write orders for medications, testing and coordinate appropriate patient referrals.
Woundtech clinicians have access to an extensive formulary of advanced dressing options.
Dressing selection is based on detailed patient and wound assessments, underlying cause, objectives of treatment, wear time, and cost-effectiveness.
Woundtech also boasts a company specific EMR that allows for robust clinical data capture and aids in treatment decision making.
As a Woundtech advanced practice clinician, you will receive the appropriate wound care training, skills, experience, and support to facilitate the assessment and management of patients based on evidence-based practice protocols to improve overall costs and patient outcomes.
Responsibilities:
Provision of hands-on care to patients as directed by Woundtech's needs.
Debridement of wounds by sharp, enzymatic, autolytic, or surgical methods completed safely during the patient visit.
Application of topical wound care and associated wound management products.
Recommendation of associated services to improve wound healing outcomes.
This includes, but is not limited to, labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
Interpretation of diagnostic tests and recommendation of appropriate follow up to same.
Recommendations for prescription of required medications and/or Durable Medical Equipment (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
Risk assessment for Members with Diabetes, as per the ADA guidelines.
Patient education and management.
Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
Provide patient updates to PCP, CM, and Physicians.

Graduate nursing degree from accredited school of Nursing and Nurse Practitioner program or Physician Assistant graduate from an accredited school for Physician Assistant education.
Possess and maintain national certification as NP or PA; wound care certification preferred.
1-3 years' experience at the advanced practice level.
Current, active, unrestricted license as a Nurse Practitioner or Physician Assistant in the state.
Ability to practice autonomously.
Ability to lift 50 lbs.
or more.
Must have full range of mobility.
Must be able to reposition patients.
Valid and unrestricted driver's license Excellent verbal and written communication skills in a professional environment.
Ability to set priorities while managing multiple tasks.
Detail oriented with excellent organization skills.
